In Spain the bogeyman is called Coco. A probable origin of the character Koko.

In the book the name KOKO is explained: It derives from a book called "Barbar the king" & "song of the elephants" from the same book.
Barbar was an elephant.
The playing cards that Koko used as his "calling cards" featured an elephant.

The song's words are:

Pata Pata
Ko Ko Ko"
