Merge proposal, October 2016[edit]

I propose that Care map be merged into Nursing care plan. Comments are welcome here. Biogeographist (talk) 16:30, 19 October 2016 (UTC)[reply]

Alternatively, and perhaps more appropriately, Care map could be merged into Clinical pathway. In fact, Care map was originally created in September 2009 by User:Jheald as a redirect to Clinical pathway, before being changed into a stub by User:Cowcilll in January 2011 for reasons that are not clear to me. I agree with the second suggestion. Nursing care plan appears to refer to guidance provided for nurses in their work, while care mapping is more akin to developing clinical pathways, which is a positive evaluation (used for cost-effectiveness, for example) rather than a normative teaching tool. As someone who worked in health informatics for 20y, I have to say I have never even heard of Care Map. But it most likely is not a Care Plan, which is something for an individual patient, rather it is more likely to be a synonym for Care Pathway, which is a structured protocol for treating a class of patient (i.e. usually a condition). Preferably, get rid of it altogether, unless an independent reference can be found to its use. Wolandscat (talk) 22:15, 21 November 2016 (UTC)[reply]

Proposal: Add new reference & content from Schrijvers paper[edit]

The paper The Care Pathway Concept: concepts and theories: an introduction. Guus Schrijvers , Arjan van Hoorn, Nicolette Huiskes should be referenced by this page, and I suggest its definition reproduced, since it is one of the clearest:

A care pathway is a complex intervention for the mutual decision-making and organisation of care processes for a well-defined group of patients during a well-defined period. Defining characteristics of care pathways include:

  • an explicit statement of the goals and key elements of care based on evidence, best practice, and patients’ expectations and their characteristics;
  • the facilitation of the communication among the team members and with patients and families
  • the coordination of the care process by coordinating the roles and sequencing the activities of the multidisciplinary care team, the patients and their relatives;
  • the documentation, monitoring, and evaluation of variances and outcomes, and
  • the identification of the appropriate resources.

The aim of a care pathway is to enhance the quality of care across the continuum by improving risk-adjusted patient outcomes, promoting patient safety, increasing patient satisfaction, and optimizing the use of resources.
