I want to know how to add a title to my submission. Also, I believe my article is suitable for posting....How do I do that after entering a Title?

I have put a {{user sandbox}} template on your sandbox page, which includes a link "Click here" to send the page to WP:Articles for creation, where an experienced user will review it and either accept it or give you feedbacks. If it is accepted, I guess they will give it the title "Connecticut Hurricanes", but the title can always be adjusted later if necessary. If you like, you could add a "Note to reviewer" to say what the title should be.
A good way to make a draft page is to click on Help:Userspace draft and fill in the intended title: that will make a draft page of the form "User:Fish1966/Articletitle", and means there is no doubt what the title should be; it also means you can have more than one draft on the go at a time.
As regards this draft, what it needs is independent references. Wikipedia's WP:Notability requirement is for references showing "significant coverage in reliable sources that are independent of the subject." Significant means more than just listing-type mentions; reliable excludes Myspace, Facebook, blogs, places where anyone can post anything; independent excludes the subject's own website, affiliated ones and anything based on press releases. The test is, have people not connected with the subject thought it significant enough to write substantial comment about? See also WP:MUSICBIO and WP:Notability (summary).
Also, an encyclopedia is not a place for organizations to tell the world about themselves, and this reads very like that. Phrases like "successful rise to prominence... take great pride in giving back to the communities... a highly regarded reputation... " are what a PR firm would write in order to boost it; but Wikipedia is very resistant to being used for any kind of promotion, and requires a WP:Neutral point of view.
