Summary

Description
English: The Swiss cheese model of accident causation illustrates that, although many layers of defense lie between hazards and accidents, there are flaws in each layer that can allow the accident to occur. The model was propounded by by Dante Orlandella and James T. Reason of the University of Manchester. This version of the image includes labels with some sample types of flaws for each layer.
